In this interactive webinar, “Discharge Planning is a Family Affair: Key Roles and Concepts for Spouse Caregivers,” Carol Levine will discuss why hospital discharge planning is important, describe federal and state laws and regulations that may affect hospital stay and discharge and costs, and offer suggestions about how spouses can be involved in decision making. The emphasis will be on understanding the hospital’s processes and priorities so that problems can be avoided at discharge, whether post-acute care is going to be provided at home or in a rehab setting. Special attention will be devoted to the CARE Act, which has been enacted in about 40 states, and is intended to prepare caregivers for post-acute care. Online and phone resources for planning will be provided to participants.
